This is the sign up sheet for the Montclair Men's Golf Association Fall Kick Off Tournament to be held on

October 16th and 17th with a 7:30 first tee time.

Format: Four man blind draw

* 80% handicap

* 1 best ball on Par 5's

* 2 best balls on Par 4's

* 3 best balls on Par 3's

* Entry fee - $50.00 - includes Tournament prizes

Once you submit your name & email address a confirmation email will be sent to you and the golf Tournament Chairman.

Your name will be placed on the sign-up sheet at the club.

Should you choose to withdraw from this golf tournament please contact a board member listed on our Contacts Page.
